Specialist Appointments on Mallorca: Waiting Times and Private Options

If you need a specialist appointment through the public health system on Mallorca, you are currently waiting longer than at any point in years. At the end of July 2026, 95,922 people in the Balearic Islands were on the waiting list for a first specialist consultation, with an average wait of 65.2 days. Surgery looks worse still: 17,139 people waiting, 107.7 days on average. If you are new to the island and seeing these numbers for the first time, it can be sobering. This post gives you the current waiting times, your rights as a patient and an honest assessment of when the private route is worth it.
At a glance
The wait for a first specialist appointment with IB-Salut currently averages 65.2 days, and 107.7 days for surgery (as of July 2026). A regional decree guarantees you a maximum of 60 days until your first specialist consultation and 180 days until surgery. If the deadline is missed, you can demand treatment at a different centre. Privately you often get an appointment within a few days, but without insurance you usually pay 70 to 150 EUR per consultation.
The lista de espera is the official waiting list of the Balearic health service IB-Salut. It records every patient waiting for a first specialist consultation, a diagnostic test or an operation, and is published monthly with figures per hospital and specialty.
Current waiting times at IB-Salut
IB-Salut publishes the figures every month in its overview of waiting list trends. The position as of 31 July 2026:
- 95,922 people are waiting for a first specialist consultation, 65.2 days on average (7.3% more than a year ago)
- 37,079 of them have already been waiting longer than 60 days, so longer than the legally guaranteed limit (38.2% more than in July 2025)
- 17,139 people are waiting for surgery, 107.7 days on average
- 3,039 people have been waiting more than six months for their operation, 53.8% more than a year ago
The load is spread unevenly: at the Son Espases university hospital in Palma alone, 6,781 people are waiting for surgery, at Son Llàtzer 3,340, in Manacor 1,857 and in Inca 1,475. Figures vary a lot by specialty, so the average says little about your specific case. Why are the numbers rising so sharply right now? The summer of 2026 was marked by several doctors' strikes, on top of the usual seasonal effect: fewer operating theatres running and staff on holiday, while the island is at its fullest. Both IB-Salut and the doctors' representatives name the strikes as the main factor behind the increase.
Your rights: the guarantee decree
Something many residents do not know: the Balearic Islands have a decree on guaranteed waiting times (garantía de demora). It sets binding maximum periods, documented in IB-Salut's regulations on waiting lists:
- A maximum of 60 days until a first specialist consultation or diagnostic test
- A maximum of 180 days until an operation
If the deadline is exceeded, IB-Salut has to offer you an alternative: at one of its own centres, another centre in the public network or a contracted private clinic. This does not happen automatically, though. You have to file the request yourself.
Check your waiting time
Count from the date your family doctor (médico de cabecera) referred you to the specialist or you were put on the surgery list. Over 60 or 180 days? Then the guarantee applies.
File the request at the hospital
Contact the Servicio de Admisión or the Servicio de Atención al Usuario of the hospital where you are on the list and ask for the guarantee decree to be applied.
Accept or decline the alternative
IB-Salut has to offer you an appointment at a different centre. If you turn the offer down, you stay on the regular list but lose the guarantee entitlement for this case.
Tip: stay reachable
Many appointments open up at short notice because other patients cancel. Keep your phone number up to date with IB-Salut and answer calls from unknown numbers too. An offer like that often comes only once.

The private alternative: what a specialist visit costs
Mallorca has a dense network of private clinics and practices, especially in and around Palma. The difference in getting an appointment is stark: instead of two months, privately you often wait just a few days, and for some specialties you get an appointment the same week.
Without private health insurance you pay for the consultation out of pocket. As a guide:

Careful: private results and the public system
A private diagnosis does not automatically replace the public waiting list. If you get diagnosed privately but want the operation done through IB-Salut, you usually have to go back through your family doctor and the public referral. Factor that in before you spend money on private diagnostics.

The guarantee decree gives you a right to an alternative treatment after 60 or 180 days, one that many people never use simply because they do not know about it.
