Convenio Especial: Public Health Insurance Without a Job in Spain

You live on Mallorca but do not work, are not employed, not autónomo, and do not receive a state pension from your home country either? Then you fall through the cracks of the Seguridad Social, because without contributions there is no automatic access to public healthcare. This is exactly the case the Convenio Especial was made for: a kind of pay-your-own-way subscription to the public health system. Many residents have never heard of it and instead pay for a private policy for years that they might not need.

At a glance

The Convenio Especial costs 60 EUR a month for people under 65 and 157 EUR from 65. You need to be registered (empadronamiento) in the Balearic Islands and prove one year of uninterrupted residence in Spain or the EU. Medication is not included; you pay for it in full yourself.

What is the Convenio Especial?

The Convenio Especial is a public insurance contract with the Balearic health service IB-Salut. For a fixed monthly fee, people who have no entitlement through the Seguridad Social get access to basic medical care in the Spanish health system, with a family doctor, specialists and hospital treatment just like regularly insured patients.

The legal basis is Real Decreto 576/2013, which sets the conditions for the whole of Spain. Each region handles the implementation; on Mallorca that is IB-Salut.

Typical cases in practice: people living off savings or rental income. Early retirees whose state pension has not started yet and who therefore cannot get an S1 form from their home country. Or a spouse without their own income who cannot be covered through the working partner.

Requirements: empadronamiento and a year of patience

The hurdles are manageable, but one of them costs time. IB-Salut requires:

No entitlement to public healthcare

Neither through the Seguridad Social nor through EU rules such as the S1 form or bilateral agreements.

Empadronamiento in a municipality of the Balearic Islands

At the time of application, proven with a certificado.

One year of uninterrupted residence

In Spain, another EU or EEA country, Switzerland or the United Kingdom, immediately before the application.

The year of prior residence is where most newcomers fall short. If you have only just moved to the island, you cannot sign up for the Convenio Especial yet and need a private solution to bridge the gap. The options for that are covered in the overview of health insurance in Spain.

If your empadronamiento has been in place for more than a year, the certificado alone is enough as proof. If you have been registered for less time, you can fill the remaining period with other documents, such as a rental contract or an earlier residence registration from another EU country.

Important for families: the Convenio Especial has no family cover. Each family member files their own application and pays their own fee, children included.

Cost: 60 or 157 EUR a month, medication extra

The fees are fixed in Real Decreto 576/2013 and have not changed since 2013: 60 EUR a month for people under 65, 157 EUR from 65. There is no medical questionnaire and no rejection for pre-existing conditions, which makes the Convenio especially attractive for older people and those with chronic illnesses, who would struggle to get private cover at all, or only with exclusions.

Watch out: you pay for medication in full

The Convenio Especial only covers basic medical care. Prescriptions from the pharmacy, prostheses and non-urgent patient transport are excluded. If you need expensive medication long term, work out your pharmacy costs before deciding. A blood pressure tablet costs little; a modern rheumatism therapy can wipe out the savings on the fee entirely.

The scope of cover is the so-called cartera común básica: family doctor at the centro de salud, specialist referrals, emergency room, surgery and inpatient treatment. During temporary stays in other Spanish regions you will be treated there as well.

Do I get a Tarjeta Sanitaria with the Convenio Especial? No. You receive a separate insurance document showing your assigned doctor and health centre, which you present together with your NIE or passport. The regular Tarjeta Sanitaria remains reserved for people with an entitlement through the Seguridad Social.

How the application works on Mallorca

Gather your documents

IB-Salut application form, original NIE or passport plus a copy, current certificado de empadronamiento showing the registration date, and, if you have been registered for less than a year, additional proof of prior residence.

Submit the application

Wait for the decision

IB-Salut decides within one month of receipt. You are then invited to sign the contract.

Sign the contract and collect your document

You sign the Convenio in two copies and receive your insurance document with your assigned doctor and health centre. The contract runs indefinitely.

Tip: pick up the form at your centro de salud

If you do not have a certificado digital, you can also get the application form on paper at your local centro de salud. The staff there can often tell you straight away whether your documents are complete before you make the trip to the registro.

When a private policy is the better choice

The Convenio Especial is cheap and takes everyone, but it is not the right answer for every situation. Private health insurance is a better fit if you are in your first year on the island and simply cannot meet the waiting period. And if you value English-speaking doctors, short waiting times for specialists or treatment in private clinics, you will not find that in the public system.

On the other hand, the Convenio really pays off from 65 onwards: private policies at that age quickly cost 200 to 300 EUR a month, with exclusions for pre-existing conditions. The Convenio takes you for 157 EUR, no questions asked. Many residents combine the two as well: basic public cover through the Convenio plus a slim private top-up policy for specialist appointments.

For a visa, though, the Convenio is usually not a solution: non-EU citizens applying for a retirement visa, for example, have to prove health insurance before entering Spain, in other words before the one-year residence period can even begin.

Conclusion

The Convenio Especial closes the gap for everyone who lives on Mallorca but gets into the public system neither through work nor through a pension: 60 EUR a month under 65, 157 EUR above, with no medical questionnaire and no end date. The two catches are the year of prior residence and the medication, which you pay for entirely yourself. Before applying, check your empadronamiento date and add up your pharmacy costs. If both work out, you get access to the same health system every other resident uses, for very little money.
